The Orleans County Office for the Aging (OFA) is one of a thirteen (13) member coalition that strives to educate and assist you with your advance care planning needs.  This process, called 'Sharing Your Wishes' helps you communicate your health care wishes to those you love, including the selection of a health care agent and completion of a health care proxy.  This is especially important when you cannot communicate for yourself.

OFA staff can assist you to consider:

What is important to you and how do you want to receive care?

Select a person to speak for you if you are unable to speak for yourself.

Talk about your health care wishes.

Put your choices in writing using the New York State Health Care Proxy form.

Everyone 18 years of age or older should have a completed proxy.
