In 2009, the Orleans County Office for the Aging (OFA) received a grant from the Community Health Foundation of Western and Central New York to help people in need* improve their transition of care from one setting to another.
OFA offers a multi-step approach, including coaching and education. One of one trained coach from OFA - Samantha Koons - begin helping by establishing a relationship between a service agency, the patient, the caregiver(s) and their family. Jim or Samantha will initially meet with the patient in their home or wherever they are located. They will help the patient and others involved in the patient's care an average of three times over 30 days, sharing information and resources to help families manage their situation.
At the end of the coaching and educational process, families should be able to make more informed decisions in their future.
* The program seeks to identify people who are 60 years of age or older who may have a urinary tract infection, dehydration, consistent falling and/or dementia.
