Training Authorization Letter

A training authorization letter is required for all NYS DHSES OFPC Courses.  Please upload a copy of your signed training authorization letter with your signup request or bring to the course on the first day of training.  The instructor has the right to refuse anyone who has not submitted a valid training authorization letter.  Download the form at the link below.
